1. That would definitely be Virginia Woolf. (Wasn't Nicole Kidman FABULOUS in _The Hours_?
2. They are all the subject of the same trivia question.
3. The covers of books like _Huckleberry Finn_ and _Tom Sawyer_. Also probably on the title pages. I've even seen it in a lot of anthologies.
